Medical Marijuana Appointment Requirements

At least one week prior to your appointment, you will need to submit copies of your medical records, either by fax, email, or bring in a hard copy. Your medical records must meet the following criteria:

  • We require 12 months of the most recent medical records (physical therapy, mental health, chiropractic etc.) that pertain to the diagnosis for which you are seeking medical marijuana licensure
  • A complete and current medication list with dosages
  • You must have been seen by your provider within the last 12 months regarding the diagnosis for which you are seeking medical marijuana licensure.

Medical Marijuana Card Process

After your appointment at Urban Unwind, you will then need to apply for your medical marijuana card through the State of North Dakota at www.health.nd.gov/mm. The state application process has a $25 processing fee. Your application will be reviewed and upon approval, you can expect your card in the mail within 1-2 weeks.

Maintenance of your Medical Marijuana Card

To maintain active licensure of your card, you must comply with the following
annual requirements:

  • A 3-month follow-up, and one yearly follow-up Urban Unwind
  • In addition to an annual follow up at Urban Unwind, you are required to see
    your Primary Healthcare Provider at least once within the 12-month span of
    acquiring your card, for the condition in which you have obtained your
    medical marijuana card.

Fees for Medical Marijuana Visits

Payment is due at the time of your appointment. We accept cash, card, Care Credit (with a $200 minimum charge), and most medical insurances. We DO NOT accept checks for Medical Marijuana visits, nor are we accepting Medicaid at this time.
